  • You must have the receipt of your last payment to the Leslie containing an invitation for the survey.
  • One must be 18 years or older to participate.
  • The survey must be taken before the expiration date mentioned on the receipt.

Steps to participate in the survey

  1. Go to Leslie’s official survey website.
  2. Read all the rules and privacy policy before proceeding.
  3. Choose the language that you prefer.
  4. There is a store code and transaction number printed on your reciept. Enter this in the appropriate box.
  5. Click on begin the survey.
  6. A series of simple questions will now appear on your screen regarding your experience.
  7. Take your time and answer all the questions honestly.
  8. Write additional feedback in the comment section or in the text box provided on the screen.
  9. After answering all the questions you will get a chance to enter the drawing.
  10. Enter the contact information asked if you wish to enter the drawing.
  11. You have now completed the survey.
